I currently collect and process the following information:

Details of any health conditions and medication (including previous serious illnesses physical and mental health).

Whether you have accessed hypnotherapy/counselling before

Frequency of your sessions

A copy of any signed contract.

Any referrals, escalations to external parties/supervisor or significant events/concerns are recorded in the case notes.
How we get the information and why we have it:
The personal information we process is provided to us directly by you for one of the following reasons:

To contact you regarding your counselling appointment times.

To send over homework tasks for you to do in between sessions, if relevant.

Assess if it is ethical to work together, in comparison to the guidelines of my governing body The National Counselling Society, available to view and read on their website at https://www.nationalcounsellingsociety.org/about-us/code-of-ethics/
Under the General Data Protection Regulation (GDPR), the lawful bases we rely on for processing this information are:

What we do with the information we have
I use the information that you have provided to help inform the approach we might take to counselling sessions.

Your contact details are not used for any other reason other than to reply to your messages or send you tasks or information as part of our work together.

The information that you provide is confidential, between you and I. I do not pass details on to any other third parties, unless required by law.

You are made aware that I discuss sessions with my supervisor, however my supervisor also works to an ethical code of a governing body for counselling and psychotherapy. What is discussed in supervision is therefore still kept confidential.

It may become necessary to share your data with a third party if I feel you, or someone else close to you, is at risk of significant harm. Unless the risk is imminent, this will be discussed with you before appropriate disclosure.

I do have a legal obligation to break confidentiality in compliance with a court order, concerns over child protection and information or knowledge regarding fraud, drug trafficking or acts of terrorism.
How we store your information:

Your personal information is stored securely in a written paper format, in a locked filing cabinet for which only I have the key. Your information is pseudonymised for further protection with a coded system that only I can understand.

If you contact me by email, ring or text me, your details are automatically saved on my devices. My devices are securely password protected.

The data information that I hold about you is stored securely for 7 years after the termination of the counselling and psychotherapy contract. After this time the information is destroyed accordingly.

Your right to rectification - You have the right to ask me to rectify information you think is inaccurate. You also have the right to ask me to complete information you think is incomplete.

You are not required to pay any charge for exercising your rights. If you make a request, I have one month to respond to you.
